Requirements for medical documents of Azimuth Airlines
Before familiarizing yourself with the specifics of ticket refunds for Azimuth Airlines, please read the article on general airline requirements. All of them must also be observed to guarantee the consideration of your request.
Medical documents include:
certificate;
sick leave certificate;
extract from medical history;
discharge summary;
extract from ambulance log;
departure airport signal sheet.
All documents, with the exception of the signal sheet, must meet the following requirements:
a clear and legible name of the medical institution that issued the document;
clearly legible position and surname of the person who issued the document;
compliance of the dates of illness and dates of contraindications to carriage, specified in the document, with the dates of the refunded carriage;
presence of a diagnosis, which may contain the name of the illness or be indicated as «illness»;
presence of at least two seals: the personal seal of the doctor (attending or chief) and the seal of the medical institution, the imprint of which must contain the full name of the medical institution.
An electronic sick leave certificate (coupon) must be certified by the seal of the medical institution and the signature — either of the attending physician or the chief physician (head of department).
Requirements for the airport signal sheet:
presence of the departure airport stamp;
information that the passenger is not permitted to depart;
passenger's surname, first name, and patronymic, indicating the flight number from which they are suspended;
preliminary diagnosis, which may be indicated as «illness».
