Requirements for medical documents of Izhavia airline
Before familiarizing yourself with the specifics of ticket refunds for Izhavia airline, please read the article with general airline requirements. All of them must also be observed to guarantee consideration of your request.
To process a refund, the airline requires the following:
passenger's application (the form will be sent by a Care Service specialist after the request is created);
copy of passport;
itinerary receipt;
two of the listed medical documents:
medical certificate from a medical institution stating the health condition on the departure date indicated on the ticket;
copy of the passenger's closed sick leave certificate, certified by the employer, or a closed electronic sick leave certificate (ESLC), certified by the seal and signature of the employer or medical institution. If a sick leave certificate is not available, an extract from the medical history or outpatient card can be provided;
certificate from the airport medical center (airport);
extract from the ambulance call log;
other document issued by a medical institution.
In case of refusal to provide a second medical document, the airline's claims commission has the right to refuse a refund.
Requirements for the content of a medical certificate:
clearly legible name, details of the medical institution that issued the certificate;
seal of the medical institution that issued the certificate;
clearly legible position and surname of the doctor who issued the certificate;
signature, seal of the doctor who issued the certificate;
signature, seal of the head of the department or chief physician of the medical institution that issued the certificate;
date of issue of the certificate;
correspondence of the illness dates indicated in the certificate with the transportation dates.
