Requirements for medical documents of Yakutia Airlines
Before familiarizing yourself with the specifics of ticket refunds for Yakutia Airlines, please read the article on general airline requirements. All of them must also be observed to guarantee the consideration of your request.
Requests for involuntary refunds due to medical reasons are not considered if the booking cancellation was made more than 72 hours before the documented date of the passenger's illness (hospitalization).
1. The following documents are considered medical documents confirming a passenger's involuntary refusal of carriage:
A. Sick leaves (incapacity for work certificates), in one of the following options:
Closed incapacity for work certificate (original or copy) certified by the signature of an authorized official and the seal of the organization at the passenger's place of employment.
Closed ELN, printed on paper and certified by the doctor's signature, the seal of the medical institution where the ELN was issued, and certified by the signature of an authorized official and the seal of the organization at the passenger's place of employment.
Coupon with the number of a closed ELN, printed on paper and certified by the doctor's signature, the seal of the medical institution where the ELN coupon was issued, and certified by the signature of an authorized official and the seal of the organization at the passenger's place of employment.
B. Original extract from medical history/discharge summary from a medical institution/hospital with hospitalization dates and the following mandatory information:
clear and legible name of the medical institution that issued this document;
details of this institution;
seal of the institution that issued the medical document;
clearly legible position and surname of the person who issued the document;
signature of the person who issued the document;
original signature and surname of the head of the department or the chief physician of the institution that issued this medical document;
diagnosis according to ICD (International Classification of Diseases);
date of onset and end of illness;
a separate entry regarding contraindications to flight must be made in the issued document;
dates of illness/contraindications to carriage must correspond to the flight dates on the refunded ticket;
date of issue of the medical document.
C. Certificates on official forms of medical institutions (original), certified by the seal and signature of responsible persons (chief physician / head of department / attending physician), as well as the seal of the medical institution, the imprint of which must contain the full name of the medical organization and mandatory information:
presence of a clear and legible name of the institution that issued the document;
presence of the details of this institution (seal with TIN and/or OGRN);
presence of a diagnosis (ICD);
clearly legible position, signature, and surname of the person who issued the document;
presence of the original signature of the head of the department or the chief physician of the institution that issued this medical document (with decryption);
presence of the date of onset of the illness and the date of issue of the medical document;
correspondence of the dates of illness indicated in the medical document to the dates of carriage, or the presence of a conclusion on contraindications to air travel during a specific period.
D. Signal sheet, extract from the ambulance call log (original).
date of issue;
patient's full name, date of birth;
diagnosis;
rendered assistance;
recommendations;
full name of the emergency medical service doctor (paramedic), signature;
seal of the medical organization.
E. Original certificate from the airport medical center:
presence in the medical document of the name of the medical center that issued this document;
presence of the medical center's seal;
clearly legible position and surname of the person who issued the document;
signature of the person who issued the document;
date of issue of the medical document;
correspondence of the date of illness indicated in the medical document to the date of carriage;
airline ticket number, flight number, departure date, date and exact time of contact with the medical center;
information about removal from the flight/suspension from flight due to medical indications.
2. If the carriage was not issued as part of a single ticket, the refund of each airline ticket is considered simultaneously, subject to the required conditions for each airline ticket.
3. The point of departure of carriage must correspond to the location of the medical organization* or, if there are no flights to the location of the medical organization, to the nearest flight point.
*If the point of carriage does not correspond to the location of the medical organization, the passenger provides a travel document to the city of origin of carriage or, if this is not possible, other supporting documents for individual agreement with the carrier.
4. If a passenger applies outside the Russian Federation to the agency that issued the ticket, the submitted medical document must be accompanied by a translation of the document into Russian or English, notarized in accordance with the legislation of that country.
5. Medical documents certified by facsimile, instead of the original signature of the doctor, head of department, or chief physician, are not accepted.
6. Medical documents with corrections, erasures, and obliterations, including those with certified corrections, are not accepted.
7. In case of refusal to fly due to medical indications, the Airline/Agent has the right to request additional medical documents from the passenger.
8. In case of suspicion of fraud, the Carrier transfers documents related to this fact to the police authorities of the Russian Federation for the initiation of a criminal/administrative case in accordance with the legislation of the Russian Federation.
