Requirements for S7 Airlines Medical Documents
Before familiarizing yourself with the specifics of S7 Airlines ticket refunds, please read the article on general airline requirements. All of them must also be observed to guarantee consideration of your request.
Refunds are made based on one of the following medical documents issued by a medical institution (state or private):
Medical institution certificate (original).
Closed sick leave certificate (original or copy, certified by the signature of an authorized official and the seal of the organization at the passenger's place of work (study)).
Closed electronic sick leave certificate (ELN), printed on paper and certified by the signature of an authorized official and the seal of the organization at the passenger's place of work (study).
Closed electronic sick leave certificate (ELN), printed on paper and certified by the doctor's signature and the seal of the medical institution at the place of ELN issuance.
Coupon with the number of a closed electronic sick leave certificate (ELN), printed on paper and certified by the signature of an authorized official and the seal of the organization at the passenger's place of work (study).
Coupon with the number of a closed electronic sick leave certificate (ELN), printed on paper and certified by the doctor's signature and the seal of the medical institution at the place of coupon issuance.
Extract from an electronic sick leave certificate (ELN), printed on paper and certified by the doctor's signature and the seal of the medical institution at the place of extract issuance.
Discharge summary, extract from medical history (outpatient, inpatient β original).
Medical commission conclusion (original).
Signal sheet, extract from ambulance call log (original).
Certificate or other medical document issued by the airport's medical station at the departure airport (original).
Other documents issued by medical institutions containing the details specified below.
All documents, with the exception of a certificate issued by the airport's medical station at the departure airport, must comply with the following requirements:
The document must contain a clear and legible name of the medical institution that issued the document (on the stamp of the medical organization or on the letterhead of the medical organization (if available), except for ELN, ELN number coupon, ELN extract).
The document must contain the details of the medical institution that issued it (address, and contact information if available).
The document must contain the clearly legible position and surname of the person who issued it.
The dates of illness or contraindications to carriage (a separate entry regarding contraindications to flight must be made in the issued document), indicated in the document, must correspond to the flight dates on the refundable/exchangeable transportation document.
The presence of a diagnosis, which may contain the name of the illness/injury or disease code, or be designated as 'illness'. For sick leave certificates, ELN, ELN number coupons, ELN extracts, indicating the illness or diagnosis is not required.
The document must contain the seals of the medical institution (with the exception of medical documents issued outside the Russian Federation):
for certificates, medical conclusions, at least two seals: the personal seal of the doctor (attending or chief β for certificates, seals of specialist doctors β for medical conclusions);
the seal of the medical institution, the imprint of which must contain the full name of the medical institution that issued the document.
for ELN/ELN number coupon/ELN extract β the presence of the doctor's signature and the seal of the medical institution, the imprint of which must contain the full name of the medical institution that issued the document;
for other documents β the presence of the seal of the medical institution, the imprint of which must contain the full name of the medical institution that issued the document.
Stamp of the departure airport.
Date of document issuance.
Information that the passenger has been denied boarding.
Passenger's ticket number or full name, indicating the flight number and date from which the passenger was denied boarding.
Preliminary diagnosis, which may be indicated as 'illness'.
7. The document must contain the date of issue.
A certificate or other medical document issued by the airport's medical station at the departure airport must contain:
Medical documents, as well as documents confirming marital status and kinship, submitted in a foreign language, must be translated into Russian, and the translation must be notarized.
According to the Air Code of the Russian Federation, close relatives and family members include β spouses, parents and children (adoptive parents and adopted children), grandparents and grandchildren, full-blood and half-blood brothers and sisters. S7 Airlines additionally includes spouses' parents (father-in-law and mother-in-law) and children of one spouse, even if the other spouse has not adopted them, as close persons.
