The basis for an involuntary refund due to medical reasons is the original medical document (certificate, sick leave certificate, extract from medical history), which must meet the following requirements:

  • the medical document must contain a clear and legible name of the institution that issued it;

  • the presence of the details of this institution;

  • the presence of the seal of the institution that issued the medical document;

  • clearly legible position and surname of the person who issued the document;

  • the certificate must have the doctor's personal seal or an electronic signature that  replaces a simple signature and the doctor's personal seal;

  • the presence of the date of issue of the medical document;

  • the date of illness indicated in the medical document must match the date of transportation. If the passenger has already been discharged at the time of the start of transportation, but flying is still contraindicated for medical reasons, an additional note in the certificate (or an additional certificate to the sick leave) from the attending physician is required stating that air travel is contraindicated during the specified period.

When the passenger provides a coupon for an electronic sick leave certificate (ESLC) or a printout of an ESLC, the document must be certified by the signature and personal seal of the doctor who issued the ESLC / chief physician / head of department or by the signature of an official (HR department / accounting / manager) and the seal of the organization specified as the place of employment, or by the employment service at the place of registration.
